ADD/ADHD Stimulant Medication Policy

If you are seeking treatment with a controlled substance / stimulant medication for ADD/ADHD, you must meet the following criteria to be accepted to the practice and to receive your prescription.

To begin treatment

  • Complete a psychiatric evaluation appointment with Serene Path Mental Health Services, LLC.
  • Complete labs, including blood work and a urine sample with a urine drug toxicology screening.
  • If your urine drug screening shows illicit drug use, you will not be prescribed stimulant medications. This includes medical marijuana and kratom.
  • Provide a previous-provider letter documenting your ADD/ADHD diagnosis, including the year of diagnosis and treatment. If you have been on stimulant medications, your previous provider must send records confirming this.

If you have never been diagnosed or treated

If you have never been diagnosed with ADD/ADHD and have never been on stimulants, you will also need to complete a QB Test — an online screening for ADD/ADHD.

The QB Test is $150.00, is not submitted to insurance, and is an out-of-pocket expense. A superbill can be provided for you to submit to your insurance for reimbursement, but Serene Path Mental Health Services, LLC does not guarantee that your insurance will cover this out-of-pocket expense.

Ongoing requirements

  • If you are on stimulant medications, you will need an in-person appointment at our Guilford, CT location on Fridays only, repeated in person every 90 days.
  • Stimulant prescriptions are for 30 days only, and a follow-up appointment must be scheduled at least every 30 days.
  • You will be asked to complete a urine drug toxicology screening at least every 90–180 days, performed at random.

Appointments

24-Hour Cancellation Policy

If a scheduled appointment is cancelled within 24 hours of your appointment, a $75.00 fee will be incurred and charged to the credit card on file.

  • If the credit card on file cannot be charged, a new credit card must be linked to the patient’s chart.
  • If no payment is made within 7–10 business days, the $75 will result in an invoice bill. If it is not paid within 30 days, the amount will go to collections.
  • No new appointment will be scheduled unless this fee is paid.
  • Three cancellations with less than 24 hours’ notice will result in discharge from the practice.

Appointments

No-Show Policy

Not showing up for a scheduled appointment will result in a charge to the credit card on file. If the credit card cannot be charged, a new card must be provided within 10 business days, and the balance must be paid within that time or the fee will go to collections. The balance must be paid in full before scheduling a new appointment.

  • First No-Show — a $75 charge to the credit card on file.
  • Second No-Show — a $125 charge to the credit card on file.
  • Third No-Show — a $150 charge to the credit card on file. After the third No-Show, you will be discharged permanently from the practice.

Practice Membership

Discharge Policy

If you are discharged from the practice under the No-Show or 24-hour Cancellation three-strike policies, the discharge is permanent and you will not be able to return.

  • Upon discharge, you will be given a 30-day supply of your prescriptions.
  • If you are on a controlled substance and have not been seen within 30 days, prescriptions for those controlled substances will not be available.
